Commemorative Brick Program
This program provides supporters of
the Museum with the opportunity to purchase a brick and leave their legacy at
this historic site.
- Please type or print information leaving one space between initials and
punctuation marks etc.
- Each Space should be counted as a character. Illegible information will cause a
delay in the brick order
- The Museum reserves the right to decline orders due to inappropriate
information. No commercial script please.
Purchases may be made either by Check (payable to: National Civil Rights
Museum) or credit card (MasterCard, Visa or AMEX) and are tax deductible. After
completing the form you may print and fax it to (901)
521-9740 or mail the form with your check or credit card

38103. Your request will be processed upon receipt of your check or
